On the Iowa DOT's website, please print off the following 2 documents:

1) Affidavit for Special Minor's Restricted License (Form 430021)

2) Parental Consent For Special Minor's Restricted License (Form 430022)

Once you have completed the required documents, please bring them to the District Office to be signed by the superintendent. 

Please allow 24 business hours for the documents to be signed and ready for pick up.
