District Forms

New Student Registration Form
Records Request Form
Open Enrollment Application for 2024-2025 and 2025-2026
School Board approval is required for students that would like to open enroll into the Saydel Community School District. The application can be sent to openenrollment@saydel.net

High School Forms

Annual Physical Form
Athletes must have a current sports qualifying physical exam on file in the Activities Office. Physicals are good for 13 months from exact date of last physical.
Concussion Form
HEADS UP: Concussion in High School Sports
Dental Screening Form
Required for all new students and 9th grade students.
Laptop Use Agreement
Must be read then complete the Laptop Acknowledgement Form for those students that have not previously completed the Laptop Acknowledgement Form.
Request for Giving Medication at School (Secondary Only)
Permissions for over the counter medication must be completed during the online registration in PowerSchool.

Woodside Middle School Forms
